•  Print Directions to the 4 nearest parking locations in the medical center so that you arrive in the office on time. 

Parking will take time, and our medical center office building has the least convenient parking. Do not rely on your GPS device. Generally we recommend parking at 6620 Main Street and walking over. Once you have reached the medical center, the process of parking and walking to our clinic may take up to 30 minutes. Please plan accordingly.

• NEW PATIENTS

1. If you are a new patient and have been referred to our practice then no forms are needed please ask your referring doctor to send your medical records. It can take 30 days to get faxed records into your chart.

2. If you are a new patient who is self referred you will need to contact previous doctors who have records might be relevant to your appointment and send us the records. 

• ESTABLISHED PATIENTS 

1. If you are a established patient wanting to send your medical records that you have with us to another doctor please fill out the "MEDICAL RELEASE TO" form located above.

2. If you are a established patient and you are wanting your records from another doctor sent to us please fill out the "MEDICAL RELEASE FROM" form located above.
