We want to be able to see your libre data when you are not in clinic. When you set up the LibreLink app, you had to create some login credentials. We''''d like you to use these credentials to log in to https://www2.libreview.com . Once you've logged in, you can open the menu at the top right, then select
1) Account Settings
2) My Practice
3) Link to a practice
4) Enter clinic ID 10043645 .
**Please let me know via portal message with Subject "LIBREVIEW" once you have done these steps so we can make sure we can see your data.

Watch this video to learn about the Librelink app you can use on your Iphone or Android smartphone. This will allow you to directly link your Librelink account to the clinic account. Your home sugar readings will be available to the Diabetes Center whenever you want to review them with us.

Go to your Iphone App store or Google Play store to download the Librelink app and get started Contact us via your patient portal when you have done all the steps.
