Patient Records

We cannot accept direct email attachments of your medical information since that is not a secure way to transmit information and several federal laws prohibit this practice. For patient convenience we have two ways in which any patient-related digital data can be sent to us.

NOTE: If sending test results that a physician at Houston Thyroid and Endocrine did not order, then the uploaded data will be placed into your chart and will NOT be reviewed until your next appointment. If you would like the physician to review the data sooner, then you will need to send a message by phone or via patient portal to let the physician know you have sent some information to be reviewed so that it can be reviewed together at a visit with the doctor.

SECURE HIPPA COMPLIANT METHODS

- Login to submit records via the Healow app (not desktop patient portal) to send medical records under your personal account (Preferred Method-limits apply) (3 day turn around average)
- Fax your information securely to 713.795.0855 (up to 15 BUSINESS DAY TURNAROUND)
- Mail to 4747 Bellaire Blvd, Suite 275. Bellaire, TX 77401. Please make sure it arrives at least THIRTY (30) BUSINESS DAYS ahead of the scheduled appointment)
