Please take a few moments prior to seeing your provider to update us on your health by filling out the Established Patient Return Form.

Reminders:

Complete the established patient return form and bring to your next visit
Review our No Show Policy and call us to confirm your appointment at least 48 hours prior to your visit. If an established patient fails to provide notice twice, it will be necessary to charge them a $25.00 fee that will be billed to his/her account.
If you are patient with diabetes mellitus who is taking insulin, you must bring your glucometer with you to your clinic visit.
We usually ask patients to complete blood work and other doctor orders a few days prior to your visit so that the doctor can discuss the results with you.
Check your test results, review your balance, refill prescriptions, schedule appointments, and update your profile on your Patient Portal
Use the patient portal to update our office on any insurance, address, or contact information changes.
