Overview
We may order a small device that tracks something important ... a scale, glucometer, blood pressure meter, or pulse-oxymeter. When it arrives just take it out of the package ... and start using it. Your device is easy ... it will automatically send its readings to our secure electronic medical record. Our healthcare team reviews your data regularly. We will call every month to encourage you and help you understand if you’re heading in the right ... or wrong ... direction. This free service makes your connection with your endocrinologist not only a great value, but a way to help him know how you’re doing between visits to his office.
Each day you take just a few seconds to use your home device. The data is sent automatically, directly and securely to your personal health record and reviewed regularly by our team. We will call you once each month with any advice to help protect your health and answer any questions you may have. This is a free service for Medicare patients . Just like your car, your body slowly changes. We can help you know when your body is changing and help you know what to do to protect your good health. This is the new science of telemedicine. We are proud to add this service as a way of keeping you healthier and happier!

Chronic care management (CCM) services may include:
At least 20 minutes a month of CCM services
Personalized assistance from a dedicated health care professional who will work with you to create a care plan
Coordination of care between your pharmacy, specialists, testing centers, hospitals, and more
24/7 emergency access to a health care professional
Expert assistance with setting and meeting your health goals
